I was excited to receive your book to learn about preeclampsia before my first doctor’s appointment and before reaching 12 weeks. However, my question about preventing preeclampsia was only partially answered in your book. During my first pregnancy, I experienced mild preeclampsia, which began at delivery (40 weeks), with slight hypertension and a slight increase in liver enzymes. Thankfully, both the delivery and postpartum period went smoothly for both me and my baby. Now pregnant with my second child, I’m uncertain about the potential increased risks for me or my baby if I take 162 mg of aspirin instead of 81 mg to prevent preeclampsia, despite its higher efficacy. Although my doctor prescribed 162 mg daily until 36 weeks, they couldn’t provide a clear answer. ACOG, SMFM, and the USPSTF recommend initiating low-dose aspirin therapy (81 mg/day) and continuing daily until delivery for individuals at high risk of preeclampsia. I want to understand if the benefits of taking aspirin 162 mg outweigh the risks, and if the higher dose should be continued until delivery.
